Most fertility specialists consider an IVF pregnancy to have reached its first major safety milestone at 10 to 12 weeks, when the miscarriage risk drops to approximately 1 to 2%. However, risk does not fall off a cliff at a single point. It drops in stages: hearing a heartbeat at 6 to 7 weeks brings the risk from around 25% down to approximately 10%; reaching week 8 brings it further to around 5%; and crossing into the second trimester at 12 weeks brings it to roughly 1 to 2%. Each scan is a milestone, and each one changes the odds meaningfully.
Why IVF Patients Ask This Question?
After the intense monitoring, medication, and emotional investment of an IVF cycle, a positive pregnancy test is not the end of anxiety. It is the beginning of a new kind of anxiety: will this pregnancy continue?
This is entirely understandable. IVF patients have been through a process that has made them acutely aware of how many things can go wrong. They are often tracking every beta-hCG number, interpreting every twinge, and waiting for the next scan with a level of attention that most naturally-conceived pregnancies never experience.
Understanding the actual risk timeline, rather than just being told “wait for the 12-week scan,” gives patients a clearer map of what each milestone means clinically.
Miscarriage Risk by Week: The Real Numbers
Miscarriage risk in early pregnancy decreases progressively, not all at once. The figures below reflect general estimates across IVF and natural pregnancies combined; age-specific figures are in the next section.

The heartbeat confirmation at 6 to 7 weeks is the single most reassuring milestone in early IVF pregnancy. Seeing a clear heartbeat roughly halves the remaining risk compared to a positive beta-hCG alone.
Is IVF Pregnancy Riskier Than Natural Pregnancy?
This is one of the most common questions asked at our Hyderabad fertility centre, and the honest answer is nuanced.
Overall, IVF pregnancies have slightly higher miscarriage rates than naturally conceived pregnancies when all patients are grouped together. However, this difference largely disappears when you adjust for age. IVF patients are on average older, and older patients have higher miscarriage rates regardless of how they conceived. The IVF process itself does not appear to significantly increase miscarriage risk when patient age is accounted for.
Miscarriage rates by maternal age (applies to both IVF and natural pregnancy):

The main reason miscarriage risk rises with age is chromosomal aneuploidy in embryos. Older eggs are more likely to produce embryos with the wrong number of chromosomes, and these embryos most commonly result in miscarriage in the first trimester rather than being carried to term.
PGT-A changes this significantly. Patients who had Preimplantation Genetic Testing (PGT-A) performed on their embryos before transfer have a substantially lower miscarriage rate, typically 5 to 10%, regardless of age. This is because aneuploid embryos are identified and not transferred, leaving only chromosomally normal (euploid) embryos in the cohort.

The 5 Key Milestones in Early IVF Pregnancy
Milestone 1: Beta-hCG Rising Appropriately (Days 10-21 Post Transfer)
A single positive beta-hCG is not sufficient to confirm a viable pregnancy. What matters is how the hCG is rising.
In a healthy IVF pregnancy, beta-hCG typically doubles every 48 to 72 hours in the early weeks. Your clinic will usually check beta-hCG at two or three time points after the initial positive test to confirm appropriate doubling.
Slow-rising or plateauing hCG suggests one of three possibilities: a biochemical pregnancy (a very early miscarriage where the embryo did not implant properly), an ectopic pregnancy (embryo implanted outside the uterus), or a pregnancy that will not be viable. Rapidly rising hCG in the expected range is reassuring but not definitive until an intrauterine pregnancy is confirmed on ultrasound.

Ectopic pregnancy risk: IVF carries a slightly higher ectopic rate than natural conception (approximately 2 to 5% vs 1 to 2%). The main reasons are prior tubal disease (a common reason for needing IVF) and the small possibility of embryo migration after transfer. The first scan at 5 to 6 weeks is partly done to confirm the pregnancy is intrauterine before the ectopic risk period passes.

Milestone 2: Gestational Sac on Ultrasound (Week 5 to 6)
The first ultrasound in an IVF pregnancy is typically done at approximately 5 to 6 weeks of gestation (3 to 4 weeks after a Day 5 blastocyst transfer). At this scan, the aim is to confirm:
- A gestational sac is present inside the uterus (not ectopic)
- The sac is appropriately sized for the gestational age
- A yolk sac is visible within the gestational sac
Seeing a gestational sac inside the uterus rules out ectopic pregnancy, which is a major relief. The miscarriage risk at this stage is still 15 to 20%, so this scan is a step in the right direction but not the final reassurance.
Milestone 3: Heartbeat Confirmed (Week 6 to 7)
This is the most emotionally significant milestone for most IVF patients. A visible heartbeat at 6 to 7 weeks, confirmed by transvaginal ultrasound, brings the risk of miscarriage down to approximately 8 to 12%.
A normal fetal heart rate at 6 weeks is 90 to 110 beats per minute. By 7 to 8 weeks it rises to 150 to 180 beats per minute. A heart rate below 100 bpm at 7 weeks is associated with a higher risk of pregnancy loss and warrants close follow-up.
If the first scan does not show a heartbeat but the gestational sac is appropriately sized, the scan is usually repeated 7 to 10 days later before any conclusions are drawn.
Milestone 4: Viability Scan (Week 7 to 9)
A second scan at 7 to 9 weeks confirms continued growth and a strong heartbeat. The crown-rump length (CRL) is measured and compared to expected size for the gestational age. Appropriate growth between the week-6 and week-8 scans is highly reassuring.
At this stage, many fertility clinics in Hyderabad begin the transition conversation: preparing to discharge the patient to an obstetric team for antenatal care. At Mamata Fertility, this handover typically happens at 8 to 10 weeks once the viability scan is satisfactory.
This scan is also the point at which multiple pregnancies (twins or higher) are definitively confirmed. If you are carrying a twin pregnancy, your care will be classified as high-risk and antenatal monitoring will be more frequent.
Milestone 5: First Trimester Scan at 11 to 13 Weeks
The nuchal translucency (NT) scan at 11 to 13 weeks is the final milestone of the first trimester and the point at which most patients and clinicians consider the pregnancy to have reached its first “safe zone.” At this stage:
- The miscarriage risk is approximately 1 to 2%
- Nuchal translucency measurement screens for Down syndrome and other chromosomal conditions
- Combined with blood tests (PAPP-A and free beta-hCG), this forms the combined first trimester screening panel
- NIPT (Non-Invasive Prenatal Testing) can also be performed from 10 weeks onwards
After the NT scan, most IVF patients are fully transferred to their obstetrician for standard antenatal care. For patients who had a particularly anxious first trimester, an additional scan at 14 to 16 weeks with an obstetrician is sometimes arranged for reassurance.
Understanding Your Gestational Age: IVF Dating Is Different
IVF patients are often confused about gestational age because they know their exact fertilisation date, which is different from how gestational age is calculated.
Gestational age in obstetrics is counted from the first day of the last menstrual period (LMP), not from fertilisation. For a Day 5 blastocyst transfer, the embryo was already 5 days old at transfer. At the time of transfer, the gestational age is approximately 2 weeks and 5 days (17 days).

Your clinic will give you an estimated due date (EDD) based on the transfer date. This EDD is typically reliable to within a few days and is used for all antenatal milestones.
Progesterone Support: When Does It Stop?
One of the most common anxieties in early IVF pregnancy is about stopping progesterone support. Progesterone is prescribed after embryo transfer to support the luteal phase and early pregnancy until the placenta takes over this function.
The placenta gradually assumes full progesterone production between approximately 8 and 10 weeks of pregnancy. This transition is called the luteoplacental shift. Before this shift is complete, the developing pregnancy depends on the progesterone support medications prescribed by your clinic.
Standard progesterone support in IVF:
- Started on the day of egg retrieval or embryo transfer (depending on protocol)
- Continued through the early pregnancy until 10 to 12 weeks in most protocols
- Form varies: vaginal pessaries (Crinone, Utrogestan), intramuscular injections, or oral tablets
- Dose and duration are prescribed by your clinic and should not be changed without their advice
Do not stop progesterone support early, even if you feel well or have had a reassuring scan. Early discontinuation before the luteoplacental shift is complete carries a risk of luteal phase insufficiency and pregnancy loss. Your clinic will give you a specific stop date based on your protocol and scan results.

Bleeding in Early IVF Pregnancy: What Is Normal?
Bleeding in the first trimester is understandably terrifying for IVF patients, but it does not always indicate miscarriage. Several causes of first-trimester bleeding are benign.
Subchorionic haematoma (SCH): A small collection of blood between the placenta and the uterine wall. SCH is more common in IVF pregnancies than in naturally conceived pregnancies and is often detected incidentally on an early ultrasound. It can cause spotting or heavier bleeding. The majority of SCH resolve spontaneously without affecting the pregnancy outcome.
Implantation-related spotting: Light spotting in the days around the expected transfer period is common and benign.
Cervical sensitivity: The cervix becomes highly vascular in early pregnancy. Progesterone pessaries, transvaginal scans, and sexual intercourse can cause small amounts of bleeding from the cervix. This is not intrauterine bleeding.
When to seek urgent review:
- Heavy bleeding (soaking more than one pad per hour)
- Bleeding with severe one-sided pain (possible ectopic, even after intrauterine pregnancy confirmed at scan)
- Passing visible tissue
- Bleeding after a previously confirmed heartbeat with significant new pain

NIPT and Chromosomal Screening: The Next Layer of Safety
Confirming that a pregnancy is continuing past 12 weeks does not mean all chromosomal concerns have been addressed. First-trimester screening offers a risk assessment for the most common chromosomal conditions.
Combined first trimester screening: Includes the nuchal translucency (NT) scan at 11 to 13 weeks plus blood tests for PAPP-A and free beta-hCG. Gives a risk score for trisomy 21 (Down syndrome), trisomy 18 (Edwards syndrome), and trisomy 13 (Patau syndrome). This is a screening test, not a diagnostic test.
NIPT (Non-Invasive Prenatal Testing): A blood test from 10 weeks onwards that analyses cell-free fetal DNA from the mother’s blood. More sensitive than combined screening for the common trisomies (detection rate above 99% for T21). Does not replace diagnostic testing but gives high-confidence screening results. NIPT is typically available in Hyderabad through major diagnostic labs and is increasingly offered as part of routine first-trimester care.
Diagnostic testing: Chorionic villus sampling (CVS) at 11 to 14 weeks or amniocentesis at 15 to 20 weeks gives a definitive chromosomal diagnosis. These are recommended when screening shows a high risk or when there is a specific known genetic concern.
Patients who had PGT-A testing on their embryos before transfer still benefit from first-trimester screening, as PGT-A screens at the single-cell level and cannot detect all chromosomal changes that may arise after implantation.
The Emotional Reality of Early IVF Pregnancy
It is worth naming something directly: many IVF patients find the first trimester of pregnancy as emotionally difficult as the IVF cycle itself. The relief of a positive test is quickly replaced by a new set of fears. Each scan is a countdown to the next one. Every day without bleeding feels like borrowed time.
This is a normal response to an abnormal level of medical scrutiny. Patients who have experienced miscarriage before IVF or during previous IVF cycles carry an additional layer of anticipatory anxiety that does not simply lift with reassuring scan results.
